Register to participate in the ECF program by
completing, signing, and returning the registration form available here.
This will open the PDF version of the form in a new window. Registration is
as easy as 1-2-3:

Save
a copy of
the form (by clicking the floppy disk icon on the
Acrobat toolbar) using your last name as the file name.

Open
the saved copy in Adobe Acrobat
and type
in the requested information. Save your completed form.

Submit
your
completed registration form.
